Registration Information


Step One:

Complete the Family/Student Form.   Annual Registration Fee of $250.00 for one child, or $500.00 per Family.  Early registration January 15th to February 16th discounted registration fees are $225.00 for one child, and $475.00 per family.


Step Two:

Fill out Authorization to Release Records.

Epiphany Cathedral School must have copies of the following items:

  1. Birth Certificate
  2. Baptismal Certificate (If applicable)
  3. Medical Insurance Card
  4. Original Copy of the student’s State Florida School Entry Health Exam (DH 3040)
  5. Original Copy of the student’s Florida Certification of Immunization (DH680)


Step Three:

Schedule a time for student placement screening for new students.

Step Four:


Complete and sign the Financial Commitment Form:


  • A nominal fee, administered by FACTS, applies when you enroll in a tuition payment plan.
